Recognizing Health Symptoms That Can't Wait Until Port

Maritime work comes with unique challenges, and access to healthcare is one of them. While some minor health issues can wait until the next port call, others require immediate attention to prevent serious complications. Vessel agents, captains, and crew members must recognize when a health concern can't be ignored. Here’s what to watch for and why acting quickly matters.

1. Severe Chest Pain

Chest pain, especially if it's sharp, crushing, or radiating to the arm, jaw, or back, could signal a heart attack. Delaying care can result in irreversible damage or death.

What to Do: Administer aspirin if available and arrange for emergency medical consultation immediately.

2. Sudden Difficulty Breathing

Shortness of breath that isn't linked to exertion or anxiety may indicate a serious issue like pneumonia, a pulmonary embolism, or a heart problem.

What to Do: Provide oxygen if the ship is equipped and contact port health services immediately.

3. Unexplained Severe Abdominal Pain

Intense, persistent abdominal pain could indicate appendicitis, gallstones, or other conditions requiring urgent intervention.

What to Do: Monitor the patient closely and seek medical advice. If accompanied by fever or vomiting, escalate the situation immediately.

4. Persistent High Fever

A fever above 102°F (39°C) that doesn't respond to medication could indicate an infection requiring antibiotics or further treatment.

What to Do: Keep the patient hydrated and isolated if an infectious disease is suspected. Contact a medical provider for guidance

5. Severe Headache with Vision Changes or Confusion

A sudden, intense headache, especially if paired with blurred vision, dizziness, or confusion, might be a sign of stroke, meningitis, or high blood pressure.

What to Do: Conduct a neurological check and seek emergency medical advice immediately.

6. Uncontrolled Bleeding

While minor cuts can be managed onboard, heavy bleeding that doesn't stop with pressure requires urgent care.

What to Do: Apply a tourniquet if the bleeding is life-threatening and seek immediate medical assistance.

7. Signs of Stroke

If a crew member exhibits facial drooping, arm weakness, or slurred speech, every minute counts.

What to Do: Follow the FAST method (Face, Arms, Speech, Time) and seek emergency medical guidance immediately.

8. Sudden Severe Allergic Reaction (Anaphylaxis)

Difficulty breathing, swelling, and hives can indicate anaphylaxis, a life-threatening allergic reaction.

What to Do: Administer an epinephrine auto-injector if available and contact medical support immediately.

9. Mental Health Crisis

While mental health issues often develop gradually, sudden signs of suicidal thoughts, extreme agitation, or disorientation require urgent attention.

What to Do: Ensure the individual is safe, provide support, and seek professional help immediately.

10. Serious Infections or Wounds

Redness, swelling, pus, or streaking from a wound can indicate a spreading infection.

What to Do: Clean the wound, administer antibiotics if available, and seek medical consultation immediately.
